COVID-19 Self Reporting Forms
Over the past few weeks, an extreme volume of new positive cases of COVID-19 have been reported to the Allegan County Health Department. The increased numbers of new positive cases have caused delays to case investigations and subsequent contact tracing.
To help achieve timely and efficient reporting, we are asking Allegan County residents only (meaning that you live in Allegan County) that test positive or have a known exposure to complete the self-reporting information below. This is a safe and secure link to provide ACHD with your information.
If you test positive, complete this “I tested positive” form.
If you have a known exposure, complete this “I have been exposed” form.
Please complete the respective form if you tested postive or have a known exposure AND have not yet been contacted by the Allegan County Health Department
Filling out the form is REQUIRED for those who want or need an isolation or quarantine letter for their employer. Individuals unable to complete the online form are asked to call the COVID-19 hotline at 269-686-4546
***If you've requested a letter from the health department, it may take up to 5 business days for us to process this request. Once we've processed the letter, we will send it to you via email.***
