Vaccine Information

The FDA has authorized the emergency use of the Pfizer-BioNTech COVID-19 Vaccine (in individuals 12 years of age to 15 years of age) and the Moderna COVID-19 Vaccine (in individuals 18 years of age and older) to prevent COVID-19 under an Emergency Use Authorization (EUA). The FDA has granted full approval for the use of the Pfizer-BioNTech COVID-19 Vaccine (in individuals 16 years of age and older).

*Minors ages 12 to 17 will need a parent or legal guardian to accompany them to their appointment in order to provide consent to receive the COVID-19 vaccine. For those 18 and older the vaccine offered depends on supply.


On the day of your appointment, please bring your photo ID/badge and wear a short-sleeved shirt. You will complete a COVID-19 screening upon arrival at the vaccine clinic. 


When you click the ACHD Vaccine Clinic links, you will see a calendar:

  1. Click on the date and choose the time that will work best with your schedule and click “Continue”.
  2. Complete the form and click “Complete Appointment”.
    1. If the link does not work try using Chrome or Firefox for your Browser
  3. You DO NOT need to enter your insurance information
